·       C - Health Information

·       G - Only if you have other life insurance or if you intend to replace life insurance.

·       H - Beneficiary

·       J - Plan Information

·       L - Premium and Billing Information

·       M - Read, Sign and Submit with application

2.    Complete the Bank Draft Authorization if paying monthly

3.    Sign and date Authorization to Disclose form

4.    Sign and date Accelerated Death Benefit Rider Disclosure form

5.    Return all forms to Green Financial:
